新一代的巨環素已經引進台灣並用於臨床上。為了解這些巨環素的抗菌活性,我們測試這些新一代的巨環素對常見臨床分離菌株之體外活性。使用agar dilution方法測試azithromycin,
clarithromycin, dirithromycin, erythromycin和roxithromycin對臨床分離菌株之最低抑菌濃度。測試細菌包括methicillin-sensitive和resistant
coagulase-negative staphylococci,methicillin-sensitive和resistant Staphylococcus
aureus,viridans streptococci,Streptococcus pneumoniae, Streptococcus pyogenes,Enterococcus
species,Corynebacterium species,Haemophilus influenzae,Moraxella catarrhalis,和Bacteroides
fragilis。結果除了H. influenzae和M. catarrhalis以外,發現大多數測試細菌之最低抑菌濃度偏高。Viridans
streptococci,S. pneumoniae,S. pyogenes,Enterococcus species,S. aureus
(methicillin-sensitive和 resistant),coagulase-negative staphylococci (methicillin-sensitive和resistant),Corynebacterium
species,和B. fragilis之MIC90≧128ug/ml。結論為在此次實驗中,大多數測試細菌對巨環素具高度抗藥性,因此合理使用抗生素是必須的。(臨床醫學
